Part II, the Supporting Documents

The forms that make up Part II of the application are due by January 15, 2013. Scroll down to the grade to which your child is applying for the required and optional forms.

All the forms are fillable PDFs. This allows you or a teacher to fill out the required form by typing directly into the form if you choose. You can save the file directly on your computer. This will allow you to work on the form and save your work and also will allow you to email the form to a teacher as an attachment. Simply right click the link for the PDF form, select "save target as" from the drop down menu, and select the file location (drive) and name to save on your computer. Completed PDFs can be attached to an email and sent to admissions@sidwell.edu or printed out and mailed to the Admissions Office at 3825 Wisconsin Avenue, NW, Washington, DC 20016.

You can also open the link and print out the forms to complete by hand. Paper forms should be mailed to the Admissions Office at 3825 Wisconsin Avenue, NW, Washington, DC 20016.
